Tracey Herlihey

Biography
Tracey Herlihey is the Head of Patient Safety Incident Response Policy at NHS England and Improvement (NHSEI) in the UK. At NHSEI Tracey is responsible for the day to day strategic leadership and subject matter expertise for the Patient Safety Incident Response Framework project. The PSIRF project outlines a new framework designed to support a strategic, risk management approach to balancing the activity and range of incident responses with those of safety improvement.
Before joining NHSEI Tracey worked at the Healthcare Safety Investigation Branch (HSIB), one of the first organisations worldwide set up to improve patient safety through effective and independent investigations that do not apportion blame or liability. Tracey Joined the HSIB initially as a National Investigator and then as Head of Safety Intelligence. She is a chartered Human Factors Specialist, Chartered Psychologist and has a PhD in Applied Psychology from Cardiff University specialising in Human Perception and Performance.
